All data is abstracted in a de-identified format so that no information will be available at any time to identify actual patients or records.

Use of Professional Judgment
AIM or Assess, Improve, Measure, and the associated quality indicators, chart abstraction questions, report features, improvement plan and impact statement, and educational resource materials are intended to be used as tools to assess practice performance. Any information presented related to basic principles of diagnosis and therapy is in no way meant to substitute for an individual patient assessment based upon the healthcare provider's examination of the patient and consideration of laboratory data and other factors unique to the patient.

Drugs and Doses
When prescribing medications, the physician is advised to check the product information sheet accompanying each drug to verify conditions of use and to identify any changes in drug dosage or schedule of contraindications.
