Gout - Develop/Approve Improvement Plan

The measure(s) you selected for improvement are available for viewing below. Based on the percentage column, set and enter a realistic goal for percent of improvement.

Reflect upon and answer the questions below, considering your plan for improvement during the improvement phase.

Click the Save button at the bottom of the screen to save data and complete the plan at another time.

Click the Submit Final Report button to submit your improvement plan and begin implementing the changes you have outlined within your practice. As noted above, the implementation phase will occur over the course of the improvement phase.

Please Note: Many of the AIM: Gout quality indicators involve newly prescribed medications; therefore, pending the number of new gout patients, it may be difficult for AIM participants to fully measure the impact of their improvements in these areas. Seeing 25 to 30 new gout patients during the improvement period may not be realistic. Please consider this during the development of the improvement plan.
